You do not need a referral to be seen at our office

Regardless of the referral status, we would like to request your x-rays, as they would be beneficial for our assessment and treatment processes.

PREPARE FOR YOUR FIRST VISIT

Please assist us at the time of your initial visit to the office by providing the following information:

Your referral slip and x-rays from your referring dentist

(In many cases, your referring dentist will have sent us this information prior to your visit)

A comprehensive medical history & a list of your current medications

(please note that you can fill out your patient registration and medical history form on our website)

If you have dental insurance, please bring any forms or insurance benefit cards

(Both dental and medical)

Prior to your virtual or in-person consultation, we kindly request that you provide us with all the necessary information mentioned above.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
